Program Outline

ENTRANCE POLICY:

Applicants must be 18 or older, and are required to have a high school diploma, GED or equivalent. Exceptions may be made if student is currently employed in a dental office. Students must provide a copy of their photo ID with application. 

ENROLLMENT:

Open enrollment for the Dental Assisting Program begins in April of each year. Registration is done by mail. Students are accepted into the program on a “first come,, first served” basis. Students will be notified by mail regarding their acceptance into the program. If student’s application is not accepted, they may reapply the following year. Students must provide a copy of photo ID with application. 

CLASS SCHEDULE:

Classes are designed for the working student. All classes are held evenings and some Saturday labs. 

CLASS LOCATION:

Lectures and labs are held at Premier Dental Care, 4411 Brownridge Terrace, Medford, OR 97504, a fully equipped, private dental office. 

OWNER, DIRECTOR, INSTRUCTOR:

Owner Kayla Burleigh an EFDA employed by Premier Dental Care and Instructor Leah Massey a Reginal Manager employed by Dentive, are graduates of SODAE and have combined 25 years of experience as well as Robert Felthousen, a very talented Radiology instructor with over 10 years of experience teaching.

CREDIT FOR PREVIOUS TRAINING:

If the student is currently, or has previously been employed in the dental field, has taken DA courses in the past, and wishes to challenge the course, he/she may do so by making arrangements with the director. Students transferring from another school must provide copies of their official transcripts. 

ATTENDANCE POLICY:

Attendance in all SODAE courses is mandatory. An excused absence will be granted in case of illness, a family death or other director approved absences. One unexcused absence will result in a warning from the director, two or more unexcused absences will result in dismissal from the course. Students are responsible for materials presented in any missed class. Late assignments must be turned in within 7 days of missed class, and will be docked 10% of grade (regardless of reason). Student must complete each course within scheduled dates. 

A leave of absence may be granted at the discretion of the director. Student must provide, in writing, a request for a leave of absence . One year is the maximum leave of absence allowed. Tardiness is not tolerated, and student will have points docked from their participation grade if they arrive late to class or leave early. 

GRADING:

Each course has required assignments. They will be graded as follows: 90-100% = A, 80-89% = B, 70-79% = C, 60-69% = D, 59 & below = F. All assignments must be complete and turned in to instructor by the last day of class. 

Students with incompletes or an accumulative grade of 59% or less in any class will not be allowed to progress to the next class. The student may retake the failed course again, the following year, however no financial credit will be given. The student must pay full tuition for the repeated class . 

PROGRESS STANDARDS:

SODAE requires that students earn a grade of A or B in Dental Assisting 1 and 2, in order to move on to Dental Radiology. All assignments are graded by the instructor and returned to the student on a weekly basis. Progress reports will be issued at 45% of each course, and at the end of DAI, DAII and Dental Radiology. Elements contributing to final grades include attendance, homework, quizzes, reports, and class final. Any challenge, or question regarding grading must be made to the instructor of the course. Students may request (in writing) a copy of their transcripts. No other person may have access to transcripts or student’s information. See FERPA 20 U.S.C.~1232g; 34 CFR part 99. 

CERTIFICATES:

Upon completion of required assignments, students will receive a Certificate of Completion for Dental Assisting and Dental Radiology. Oregon licensing for Dental Radiography and EFDA require passage of Dental Assisting National Board (DANB) exams. See their www.danb.org for more information. 

DISCIPLINARY POLICY:

Students are expected to be a credit to themselves and SODAE. All school rules are to be followed. Director maintains the right to dismiss students in violation of school policies or appropriate conduct. 

CONDUCT:

Enrollment with SODAE carries with it certain responsibilities regarding standard of conduct. 

Students are expected to practice academic honesty. Students must not cheat, plagiarize, steal course materials or tests, furnish false information, forge or alter SODAE information. Punishment for such behavior will be dismissal from course. 

Students must show respect to each other and course instructor. They must not threaten the safety of others, or disrupt the educational process. 

Students must show respect for properties used for instruction (classrooms, lab facilities). They must not steal or damage course materials or facilities, and will be held financially responsible for any such damage or theft. 

Students must not possess or be under the influence of alcohol, marajuana, illegal or controlled substances while attending class. 

Any violation of the above policies will result in a written warning, second violation will result in dismissal from the program. No probation is offered. Students violating the above policies will not be allowed to reenter the program.

STUDENT GRIEVANCE POLICY:

Students aggrieved by the action of the school should attempt to resolve these problems with appropriate school officials. Should this procedure fail students may contact: Higher Education Coordinating Commission, Private Career Schools, 255 Capital St. NE, Salem, OR 97310. After consultation with appropriate Commission staff and if the complaint alleges a violation of Oregon Revised Statutes 345.010 to 345.470 or standards of the Oregon Administrative Rules 715-045-0001 through 715-045-0210, the Commission will begin the complaint investigation process as defined in OAR 71-045-0023 Appeals and Complaints.

TUITION AND FEE POLICY:

Tuition is due by July 15. SODAE accepts personal checks, bank checks, money orders or cash. We can accept credit cards however there will be a 3% processing fee added to your balance. If interested in using a credit card just let the director know and we will get you set up to do so. Because SODAE is such a small school, financial aid is not available. No discounts, waivers, or scholarships are offered. We do have a payment plan option if you wish to choose it.

Payment Plan: Upon registration one quarter of the tuition ($1750) is due along with the $50 application fee (total $1800). The second payment ($1750) is due at the end of Dental Assisting 1 (no later than October 8, 2020). The remaining balance ($3500) will be due at the end of Dental Assisting 2 (no later than December 10).

CANCELLATION POLICY:

Students may drop and receive full refund provided student gives written notice within 5 days of signing enrollment agreement, and before classes begin. Registration fee and book fees, however are non-refundable. 

After the commencement of class, a full refund is not given. If written withdraw or termination occurs after instruction has begun, refunds will be prorated based on percentage of class attended. Cancellation is effective following last class student attended. No refund will be given after 50% of instruction is offered. SODAE will retain the registration fee and fee for texts.
