Application Details
The completed application with the Class Application, Enrollment Agreement must be filled out and submitted by mail to (P.O. Box 1383, Jacksonville, OR 97530), along with a copy of a valid driver’s license, a high school diploma or GED, a $50 application fee, and the tuition payment based on your selections. If applying for the 3 payment SODAE payment plan, please also fill our the Contract for the payment option.
Applications may also be submitted by email to (kburleigh@sodaeprogram.com). In that case, we will send an invoice for the $50 application fee and the tuition payment (based on your selections) to the email address provided on the form. Please note that a 3% card processing fee will be added to all invoices.
