ENROLLMENT REQUIREMENTS:
Students must be 18 years of age or older, and provide proof of High School Diploma or GED, or are currently employed by a dentist. Exceptions to requirements may be made at the discretion of the school director. SODAE has an open-door policy for enrollment. Students are enrolled on a “first come, first served” basis. SODAE staff do not discriminate based on race, religion, color, national origin, age, gender, sexual orientation or disability.
CANCELLATION POLICY:
Students may drop and receive full refund provided student gives written notice within 5 days of signing enrollment agreement, and before classes begin. Registration fee and book fees however are non-refundable.
After the commencement of class, a full refund is not given. If written withdraw or termination occurs after instruction has begun, refunds will be prorated based on percentage of total classes attended. Cancellation is effective following last class student attended. No refund will be given after 50% of instruction is offered. SODAE will retain the registration fee and fee for texts.

RETURNED CHECKS:
A $50.00 returned check fee is charged for any returned check. Tuition amount plus returned check fee must then be paid by cashier’s check, money order, or cash and no personal check will be accepted for tuition in the future.
CONDUCT:
Enrollment with SODAE carries with its certain responsibilities regarding standard of conduct.
1. Students are expected to practice academic honesty. Students must not cheat, plagiarize, steal course materials or tests, furnish false information, forge or alter SODAE information. Punishment for such behavior will be dismissal from course.
2. Students must show respect to each other and course instructor. They must not threaten the safety of others or disrupt the educational process.
3. Students must show respect for properties used for instruction (classrooms, lab facilities). They must not steal or damage course materials or facilities and will be held financially responsible for any such damage or theft.
4. Students must not possess or be under the influence of intoxicants, illegal or controlled substances while attending class.
ATTENDANCE:
Attendance is mandatory for all SODAE courses. SODAE understands and recognizes that emergencies do occur. Students are responsible for contacting the instructor regarding any missed class. Make-up classes will be offered at instructor’s discretion, based on SODAE’s attendance policy listed in the school’s current catalog. Students are responsible for course materials missed during an absence and must make arrangements to get materials from the instructor or another student.
ASSIGNMENTS:
Most SODAE courses have required assignments. In order to pass the courses, or participate in activities such as finals, clinical exams, report presentations, students must turn in all completed required assignments. Students who have missed a class have 7 days to turn in required assignments. Assignments turned in on or after the 8th day will receive a 0. If an assignment is returned to student with an “Incomplete”, the student has an additional 7 days to complete the assignment and return it to instructor. Again, a 0 will be given if incomplete assignment is turned in on or after the 8th day, or not returned to instructor. Two or more zeros in the grade book are grounds for dismissal from course. Assignments may be sent to instructor via email (without penalty), as long as they are received by 6:00 pm on date due.
SIGNATURE STATEMENT:
My signature indicates that I am at least 18 years of age, that I have read this document and the school brochure, agree to any stipulations listed within both documents, and have received a copy of this document. I understand that this enrollment agreement is a legal and binding contract.
